Resident / Attending staff
Inclusion criteria (all of below)
- Clinical diagnosis of Ischemic stroke
- Onset of symptoms <4.5 hours before beginning treatment
- Appropriate for rt-PA administration as per exclusion criteria
- Age ≥ 18 years
- Non-contrast CT brain shows no hemorrhage!
- Informed consent
Absolute Exclusion criteria (any one of below)
- Presumed septic emboli, bacterial endocarditis
- Serious head injury, intracranial or intraspinal surgery in previous 3 months
- Clinical CT brain, suspicion of subarachnoid hemorrhage
- History of previous intracranial hemorrhage
- Intracranial neoplasm
- SBP > 185 or DBP > 110 mmHg despite adequate treatment
- Active internal bleeding; GI/GU tract bleeding, within previous 3 weeks
- Acute bleeding diathesis, including but not limited to
- Platelet count < 100,000 /mm3
- aPTT elevated greater than upper limit of normal for laboratory
- INR> 1.7 or PT >15 seconds or received a dose of LMWH within the previous 24 hours
- CT shows multilobar infarct (hypodensity > 1/3 cerebral hemisphere)
- Current use of direct thrombin inhibitors with elevated aPTT or TT or ECT in patient with ClCr > 50 ml/min
- Current use of direct thrombin inhibitors with last dose of direct thrombin inhibitor < 2 days in patients with ClCr > 50ml/min
- Current use of direct thrombin inhibitors with ClCr< 50 ml/min
- Current use of direct factor Xa inhibitors with elevated aPTT or PT or INR or abnormal factor Xa activity in patients with ClCr > 50 mL/min
- Current use of direct factor Xa inhibitors with last dose of direct factor Xa inhibitor < 2 days in patients with ClCr > 50 mL/min
- Current use of direct factor Xa inhibitors with ClCr < 50 mL/min
Relative exclusion criteria for ONSET <3 hr :
- Female patient who is possibly pregnant or during pregnancy or lactation
- History of recent stroke or MI within previous 3 months
- Major surgery or serious trauma within previous 14 days
- Lumbar puncture or arterial puncture at a non-compressible site in previous 1 week
- Glucose < 50 mg/dl
- Only minor (NIHSS < 4) or rapidly improving stroke symptoms (clearing spontaneously)
- Seizure at onset with postictal residual neurological impairments
- Intracranial AVM or cerebral aneurysm
Relative exclusion criteria ONSET 3-4.5 hr :
- Aged >80 years
- Severe stroke (NIHSS> 25)
- Taking any oral anticoagulant regardless of INR
- History of both diabetes and prior ischemic stroke
- Only minor (NIHSS < 4) or rapidly improving stroke symptoms (clearing spontaneously)
- Seizure at onset with postictal residual neurologic impairments
- Female patient who is possibly pregnant or during pregnancy or lactation
- History of recent stroke or MI within previous 3 months
- Major surgery or serious trauma within previous 14 days
